How it works

The form is fixed. Only the answers change.

Word proformas are painful because the layout is the document: every keystroke risks a broken table or a signature box pushed onto the next page. Clinical Forms separates the two. The layout is fixed and owned by us, you only ever touch the values, and the AI pre-fills those values from a conversation that already happened.

Record the appointment as you normally would. The assessment or consent conversation you are already having out loud is exactly what the form needs.

On the template step, open the Forms tab and choose the form. A draft is created from that conversation straight away.

Review every field. Anything not discussed stays blank and is flagged, never guessed. Edit anything you like.

The exact layout, every time

The form is drawn by us, not by the AI, so the same form comes out identically on every export: headings, grey guidance boxes, Yes/No columns and signature blocks exactly where your Trust put them.

Records only what was said

Fields are filled from the recorded conversation and nothing else. The AI does not add clinical detail, does not guess at blanks, and quotes the person’s own words where they were spoken.

Flags what is missing

Required fields and statutory questions with no recorded answer are shown as warnings before you can approve. On the MCA that includes each of the four functional test questions.

Fill in without a recording

No transcript? Open the form blank on screen with the standard wording already in place and complete the variable parts by hand, which is a fair bit quicker than a Word document.

A summary for your notes

After approval, AI creates a few sentences from the approved form only, ready to paste into your practice software as a single entry. The signed form stays the record.

Does the AI decide whether a patient has capacity?

No. Clinical Forms records what the assessor stated during the assessment. It never assesses, suggests or concludes whether a person has or lacks capacity. Questions the assessor did not answer stay blank and are flagged for the clinician, and nothing is filled in on their behalf.

Which forms are available today?

The Hertfordshire Multiagency Mental Capacity Assessment with Best Interests Decision Record, and NHS Consent Forms 1 and 2 in general anaesthetic and inhalation sedation versions with the standard benefits and risks wording built in. Hospital referral proformas and other Trust forms are added on request.

Can I add my own Trust or hospital forms?

Yes. From the Forms tab in AI Notes, send us the blank form your Trust, hospital or practice uses. Every form is built and checked by hand so the layout and wording match your document exactly, and we let you know when it is ready. Once built, everyone in your service can use it.

What if I did not record the appointment?

Choose Fill in without a transcript. The form opens blank on screen with all the standard wording in place, you complete the variable parts, approve, and download the same PDF or Word file. No AI is involved on that path.

How does it fit with R4, SOE or Dentally?

The completed form downloads as PDF or Word to print, sign and file as you do today. After approval you can also create a short summary of the outcome and paste it into your practice software as a single notes entry.

Is the audio stored?

No. Audio is processed to create the transcript and then discarded, the same as the rest of AI Notes. Saved forms stay in your account until you delete them.
