Safety and Data Protection keeps three registers together: events, safeguarding concerns, and data protection. Anyone in the team can record an event, from a near miss to a critical incident, and things that went well too. They say what happened and what was done, and your managers are alerted.
An admin reviews each one, and can change its type or severity, or raise it to a critical incident. If it needs more, start an investigation: the findings, the root cause and the outcome. Record whether Duty of Candour applies, and whether it's notifiable to the CQC or reportable under RIDDOR.
Give each corrective action an owner and a deadline. They're told, and reminded if it runs late. The event can't close until every action is done, and every step stays in its history. Safeguarding concerns have their own register. If anyone is in immediate danger, the form reminds staff to follow your emergency procedure first.
Staff give a factual account and submit it securely. It's sealed straight away, and they get a reference. Only the leads and deputies you name can open it, and they're alerted with the reference, never the details. Leads record the risk, referrals, actions and outcome, and the access log shows who has opened the case.
Subject access requests and personal data breaches each get a statutory deadline: one calendar month, or seventy-two hours. The lead is alerted as a deadline gets close, and a breach can't be closed until the ICO decision is recorded. Anything overdue shows on your Practice Hub, and every register exports as a spreadsheet when you need it.
